California Consumer Privacy Act
The California Consumer Privacy Act (CCPA) outlines specific requirements for companies that correlate with consumer rights over their personal data. The following information reflects our processes at Haley Music Therapy in relation to the CCPA requirements.
- Right to Disclosure. The only data collected from our users would need to be provided directly by the user. If a user fills out our “contact form” or “request a quote form”, the information that they provide will be emailed to our staff and used to contact you, respond to your inquiry, and provide services in alignment with our business.
- Right to Access and Contact Information. At any time, our clients may request a copy of all information that we have on file for them. To request a copy of any documentation related to you or your child, please email erin@haleymusictherapy.com and indicate if you would prefer a digital copy or hard copy mailed to you. For privacy reasons, we may require photo ID in order to receive your file. We are legally required to respond within 45 days of your inquiry and disclose all information in your file.
- Right to be Forgotten. You have the right to request that we delete any and all personal data from our system. Please submit your request by emailing erin@haleymusictherapy.com.
- Opt-out of Data Sales and Marketing. Haley Music Therapy will never sell your information or share your information with a third party unless required by law or given permission by the client.
How is Client Information Stored?
For digital files/documentation Haley Music Therapy uses Google Drive. HMT has a signed Business Associate’s Agreement with Google, making our system HIPAA compliant. Paper files/documents are stored in a locked closet, separate from non-confidential files/documentation. (This will change when we get the client portal up and going).
How Do We Use the Information That Clients Provide to Us?
We use this information to provide services to our clients. We do not sell or share personal information without express consent from the client. Examples of when we may request to share information is to communicate with client team members such as SLPS, OTs, PTs, doctors, etc.; to share progress reports with an educational institution or regional center; or if a client chooses to participate in community-based activities, such as Community Performances.
